One patients from the means by which the people infected with the H7N9 avian influenza infection? Whether the close contacts of morbidity or mortality? Close contacts whether they are under medical observation?

Answer: the people infected with the H7N9 influenza virus from the virus biology belonging to the avian influenza virus, previously found in the international human infection H7 subtypes of influenza virus are mostly from birds, but so far, three cases of diagnosed patients The specific source of infection is not clear.

According to the relevant provisions of unexplained pneumonia Management, Shanghai, Anhui and Jiangsu, the health sector (1 patient in Anhui where Nanjing) all close contacts of patients implemented under medical observation. The Shanghai patients diagnosed patients prior to the onset, the two family members with severe pneumonia, the cause is still under investigation and detection. 3 patients the incidence of the family association, pending further investigation. In addition, Shanghai, Anhui diagnosed patients of all close contacts were not found in similar cases.

Second, the National Health and Family Planning Commission, as well as local health departments to take measures? Which investigations were carried out?

The cases of the disease, local health departments attach great importance to take timely clinical treatment, epidemiological investigation, specimen collection and submission, medical observation of close contacts, and to strengthen the monitoring of unexplained pneumonia cases.

National Health and Family Planning Commission attaches great importance to this epidemic, received severe unexplained pneumonia outbreaks have been reported, immediately sent a working group of experts to Shanghai, Anhui and Jiangsu, there to supervise the patient's treatment and investigation, to convene a multisectoral Chamber of Commerce will the epidemic nature of the analysis and judgment, and arrangements for the investigation, prevention and control and preparedness work. China CDC received case reports and submission of specimens in a timely manner to carry out the epidemic risk judgments and laboratory testing research, clear etiological diagnosis.

Third, the two cases diagnosed patients died in early March, why the state Health and Family Planning Commission, released March 31, the epidemic?

A: The people infected with the H7N9 avian influenza virus, is the world's first discovery of a new subtype of influenza virus, has not been included in our statutory report infectious disease monitoring and reporting system.

Medical and health institutions in Shanghai found that the above-mentioned cases, in a timely manner to carry out the relevant laboratory screening, has ruled out the infected with seasonal H1N1 and H3N2 influenza, H1N1 flu, people infected with the highly pathogenic avian influenza (H5N1) and SARS, new Coronavirus possible. In further testing, the Shanghai Public Health Clinical Center on March 22 found that patients may be infected with the H7 influenza, after specimens were sent to the China Disease Prevention and Control Center. On the afternoon of March 29, the Chinese Center for Disease Control three H7N9 avian influenza virus isolated from the submission of cases specimens. National Health and Family Planning Organization experts further verification, according to laboratory test results, combined with the clinical manifestations, epidemiological data to conduct a comprehensive analysis to determine the patients with human cases of infection with the H7N9 avian flu virus. National Health and Family Planning Commission, in accordance with the law to the public on March 31.

Fourth, after media reports that a family of three infection unexplained pneumonia, two of whom died, whether related to the reported cases?

February 20-25, in a hospital in Shanghai has treated three people suffering from severe pneumonia, surnamed Li and his son, two people died. Mr Lee, his father was diagnosed with the H7N9 avian influenza virus infection, the diagnosis of two patients in progress.

H7N9 virus with previous H1N1, H5N1 and other seasonal influenza cases compared to its virulence and infectious?

Currently only found three cases of human infection with the H7N9 avian flu virus confirmed cases of the virus and its diseases caused by very limited research data, expert judgment is that the virulence of the virus and the human ability to spread further.

Sixth, if the infection early symptoms seek immediate medical attention if you can get effective treatment?

The gene sequence analysis showed that the drug-sensitive virus neuraminidase inhibitor class of anti-influenza virus. Experience other type influenza antiviral treatment after the onset of the early use of the neuraminidase inhibitor class of anti-influenza virus drugs may be effective remains to be seen, but the means of specific treatment of newly discovered human H7N9 avian influenza virus infection research.

How to prevent the H7N9 avian flu? Are there any other special precautions?

Influenza is an acute respiratory tract infection. Wash their hands, indoor Qin ventilation, attention to nutrition, to maintain a good physique is conducive to the prevention of influenza and other respiratory diseases. Respiratory tract infection symptoms such as sneezing, coughing, use a tissue, handkerchief Cover your mouth and nose to prevent infection of others. In addition, special attention to avoid direct contact with sick or dead poultry and livestock.

More contact with patients suffering from infectious diseases than the general public due to the medical staff, it is recommended that the medical staff to take the necessary protective measures in the process of the treatment of the patients. Admissions with suspected or confirmed H7N9 avian influenza, effective protective measures should be taken, that the standard prevention, prevention of airborne droplets and contact transmission precautions.

At home and abroad there is no vaccine against the H7N9 avian influenza virus.

【Beijing the H7N9 virus into infectious disease surveillance system]

I feel motivated to once again voice my opposition to the importation and purchase of any made-in-China pet foods, especially those desiccated jerky type doggy treats.

It is odd to have it jump species so quickly and in so many places at once. Would seem it has been simmering below the radar for some time.

For sure. It's now been found in Shanghai, Anhui, and 4 different cities in Jiangsu. Like suddenly annoucing a case in LA, 2 in San Fran, and 4 in various parts of Arizona.

Does it mean H2H transmission? I guess it doesn't prove it, but boy things are looking odd.

This outbreak, now in 7 different cities, is either:
1) H2H
or
2) not H2H, which means there are infected birds/pigs in 7 different cities in china, so it has been spreading so long among animals that it's now endemic in birds/pigs in SE china.

It can easily be widespread in chickens, we saw that in too in 2005.

The cases are still all individuals in many locations?

Now we watch for reports of clusters of related cases, especially those with staggered onset dates. Or just many more cases popping up at the same time.

Does anybody know when the usual Chinese new year holiday ended? I think it's 3 weeks or a month or so and it's usually in march. Lot's of people travelling together then. Can't be too long ago...

Also, it would seem that nature may have picked a lock that was artificially picked in the hotly debated H5N1 studies:

Quote:
All three of the H5N1 transmission studies introduced the receptor bind domain change Q226L to increase affinity for mammalian receptors.

Q226L has never been reported in natural H5N1 isolates.

Two of the H7N9 cases, A/Shanghai/2/2013 and A/Anhui/1/2013 have Q226L, raising serious pandemic concerns.

If this is "it", not long at all. A week, maybe two. Fingers crossed it is not a pandemic strain.

There is a POSSIBLE family H2H case but documenting it probably isn't going to happen.

Patient #3, Ms. Han from Chuzhou City, Anhui Province, has a daughter who was sick with a "cold" or ILI about 2 weeks before Ms. Han was hospitalized. The onset dates are a bit vague so it's hard to know if the daughter's illness is actuallyl an issue or just a coincidence. I haven't seen any reports that specify whether the daughter was tested or not - only reports that "close contacts" are being monitored.

Patient #1, the 87 year old man, also had 2 sons who were ill. One developed severe pneumonia (IIRC), the other had only relatively mild symptoms. But both of the sons tested negative for H7N9. So even if the daughter of Ms. Han did have a mild case of the virus, she may not test positive either.

So the four 'new' cases developed symptoms between March 19-21. The typical incubation period range for influenze is 1-4 days with 2 days being average. It's thus far claimed that no close contacts have come down with symptoms. I'm going to be 'generous' & assign this one a 3 day incubation period - because I can & because we don't know. That means those four 'new' cases, if they're transmitting H-H have had almost 4 potential generations of transmission & IF this is efficient enough to cause problems.... we're going to find out reeeeeeeeeeeeeeeal soon what's really going on.

What's going on in the Yangtze River Delta.

Bird Flu Mystery Recalls Chinese SARS Coverup

Quote:
ortunately, the odds are that the 2013 bird-flu outbreak will turn out to be not much of a crisis at all. But that’s little consolation to Chinese wondering about their odds in a pandemic. This evening, H7N9 is a top trending topic on Sina Weibo, with many voices openly doubting the public information about the virus and the speed at which it was disseminated. If the Chinese government hopes to cool the nerves of its netizens, it’d be well-advised to remember the lessons of SARS before it’s too late to act on them.

Excellent article.

BTW, as many of us know, Laurie Garret is a top notch journalist. I trust her opinion.

WHO Director: H7N9 may be asymptomatic in birds&pigs, appears to have partially adapted to humans through re-assortment

http://www.thestandard.com.hk/breaki...l.asp?id=34300
“We speculate that when this virus is maintained in poultry the disease will not appear, and similar in pigs, if they are infected, so nobody recognizes the infection in animals around them, then the transmission from animal to human may occur,'' said Dr Masato Tashiro, director of the World Health Organization's influenza research center in Tokyo and one of the specialists who studied the genetic data.
...
Other information gleaned from the genetic data was that the H7N9 virus was what scientists call a “gene re-assortant’’ _ in which three bird viruses swapped genes among themselves _ undergoing changes that allowed it to adapt more easily, though not fully, to human hosts, WHO's Tashiro said. One change has allowed it to lodge onto the surfaces of cells of mammals, making it easier to infect humans.
“The tentative assessment of this virus is that it may cause human infection or epidemic. It is still not yet adapted to humans completely, but important factors have already changed,'' Tashiro said.
